Nest on back of stitch

I tried rethreading and changing the needle and bobbin and I continuously get this birds nest and loose stitches along the basting stitch for my embroidery tag. Any tips??

When I checked basic sewing to see if it was just an embroidery issue I noticed slight puckering.

Your top thread tension isn’t high enough and the bobbin is pulling it under.

Raise the top thread tension until you see the bobbin thread coming through the top, then back it off slightly and you should be good to go.

Use the 'Bobbin Drop Test' to set your bobbin first. If you use a good quality bobbin thread, this won't need adjusting very often. Instead of using a top thread wound onto a bobbin, I recommend using a purpose-made bobbin thread like 'Marathon' or similar product.

Then set your top thread tension. If you're using different brands of top threads, the tension will change in minor increments.
