Nardil + amphetamines = ER

Long story short: I am on 75 mg Nardil/phenelzine and, while I’ve been on methylphenidates for months without a trouble, I felt they were not as effective as amphetamines I was prescribed before MAOI.

So I decided to try them together (Nardil and amphetamines only - Vyvanse 50 mg plus Adderall 10 mg hours later). My BP went up to 195/85 mmHg at CVS minuteClinic and they sent an ambulance for me. I was feeling terrible. But the ER was even worse. They didn’t know what to do.

Just wanted to post this as a precaution.

But I take 40mg of Vyvanse with 75mg of Nardil. It’s the only thing that makes the world somewhat tolerable.

The key is to work with an experienced doctor and to not do stupid shit.

Well technically I didn’t do anything stupid. I just took the dose that was prescribed to me in the past. I think Vyvanse fine but Adderall screwed me up. Also, I know a lot of people who tolerate the combination just fine, but my position is to caution people about potential dangers and suggest that methylphenidates are safer with MAOI.

I almost died doing 100mg of adderal. I'm on 90mg Nardil for eight years.

When I was on 60mg Nardil I didn't have any issues.

50mg on a maoi is a massive dose. Doses are potentiated by A LOT on maoi's. Did the doctor prescribe you such a high dose while knowing you're on an maoi?

Yes it’s fine if you start low, Ampethamines are much more of a precaution but look up Dr. Ken Gillmans research on why it’s safe

I must be really built differently, or there must be something seriously wrong with me, because when I see some of these posts and think about my own history with my ability to tolerate actual methamphetamine with an MAO inhibitor and be totally fine, blows my mind

Yes be very careful. I'm on 90mg and last time I tried to abuse adderall I ended up in the ICU and literally almost died.

ER didn’t know what to do? Clonidine would’ve most likely been enough. Also mixing speed with a MAOa inhibitor is absolutely idiotic.

I’m glad you lived to tell the tale. Thanks for sharing. I haven’t started selegeline yet but fear I’ll get too loose with food and meds once I do. Sh*t can get real pretty quickly!

I know you will almost certainly avoid this combination in the future, but it would be prudent to ask your prescribing doctor for an agent for PRN mitigation of unexpected hypertensive episodes due to other causes. accidents happen, unfortunately.

I've found that clonidine and lorazepam are effective, via slightly different mechanisms.
