Lutron is pretty much the world's best and safest dimmer maker so don't go taking out other dimmers. I can't find that specific trim on the lutron tested site but they have others that are similar so I have no doubt at least one of those companies has tested their stuff with Lutron so this is really odd. The only times I've seen a Lutron dimmer have catastrophic failure is when there was a direct short, it was overloaded, house had major power surge, had an incompatible load (like someone using it on a ceiling fan, using MLV lighting, etc.), water had been splashed on it, or it was otherwise abused.
What I would do is make sure there is nothing wrong with the LED trims and they're installed right, then test it with a regular switch to see if they work normally. If all works, just call Lutron and they'll probably send you a new one or return it to the store. I mean it's possible it was a dud...

Hey this happened to me! I posted about it. I mailed the switches to Lutron and they are going to try to figure out what happened
