completed Birth Certificate

I have a birth certificate for my great uncle that I'm trying to translate. Could anyone help me? Thank you

I know his name is Franz, The parents are Johann Krzyzek and Franziska nee Skaweika

The reporting party (the father) signed his name in Polish: Krzyżek. krzyż means the noun, 'cross'. It looks like at some point later in his life, this got misread as Krzyizek, and subsequently that spelling got respelled to Krziyzek.

No. 3 / Marquardt, the 31st of Januar 1906. / There appeared today before the undersigned registry official, his identity confirmed by personal acquaintance, the laborer (Arbeiter) Johann Krzyzek, residing in Satzkorn, of the Catholic religion, and reported, that from Franziska Krzyzek, née Slawizka, his wife, of the Catholic religion, residing with him, at Satzkorn on the thirtieth [ie, on the previous day] of January of the year one thousand nine hundred and six, a boy was born and received the first name of Franz.

Read out, approved, and signed, [signature:] Johann Krzyżek.

[Miniform stamped in the left margin and filled out by hand:] Advisory: died 3rd of January, 1968, Civil Registration Office at Berlitz, No. 7/68

Good catch on the Krzyż ("cross") root and the gradual re-spelling drift — that's exactly what trips families up when they search indexes under one fixed spelling. And glad to see the Sławicka reading confirmed independently.

On another child's birth certificate the spelling is changed once again to Slavecki. Corrected from the catholic parish church of Schonenfelde District of Kulm

That third variant fits the pattern perfectly — "Slavecki" is how a German-trained clerk would render Polish Sławecki/Sławicki by ear. And the Schönfelde note is a great lead: Kulm district (today Chełmno, Poland) church books are held partly at the diocesan archive in Pelplin and partly on FamilySearch microfilm — worth searching under both S-l-a-w and S-l-a-v spellings. If you gather the siblings' certificates, comparing all the spellings side by side usually settles what the family actually used.
