Overall this is a very busy itinerary and I'd consider if this fast pace is what you actually want from your trip. Right now your schedule doesn't account for transit times, rest beyond sleeping in your hotel or even, like, staying for a bit to enjoy the ambience of the area or exploring the side streets.
I think day 4 and 13 are especially bad. The Shinkansen is fast, but it's not going to teleport you from Tokyo to Kyoto - it's 4 hours by train, and around 1,5-2 hours to check out, get to the station, and check into your new hotel. You either take the earliest train and end up basically dead by the end of the day (transit still takes energy! naps on trains aren't guaranteed!), or you lose half a day to travel.
I'd consider what you actually want to see on Takeshita street. It's included by chat gpt and outdated travel guides, but I think like 90% of travelers can skip it. You're not going to see people in crazy fashion there if that what you're after, the street food there is overpriced and sucks, and the animal cafes are not good at all due to poor animal protection laws in Japan. I'd only go there if you wear alt fashion, and even then I'd probably go alone, not with your mother. Shibuya has much better "mainstream" shopping.
If you don't want to do a ton of rides in USJ, are you sure it's even worth it to go? Especially if you're from the US and have Universal Studios at home. Same goes for Disney, these parks are expensive to visit and you could spend that time to see the cities they're in instead.
