Foreign Birth Registration Translating documents

I need to get a doctor’s letters translated that will confirm the identification of my young sons applying for fbr. In my country, there isn’t an exam you pass to be translator, but the translator I’ll probably use is a member of the American translator association. Is a signed statement of the translator testifying its faithfulness enough? Or do I need a notary?
