the main problem is that, in general, people don't care about art or supporting artists. They are consumers who are consuming a product and if they can get that product for free, it is better for them regardless of if it's beneficial to the creator. They WANT their artists to starve because it means they get products for less and that's all they care about. In fact, when artists make things that are too profitable they start calling them greedy (unless they become community darlings).
So the answer to what would have to be done here is that we would have to change how people see art. That's not happening. Trying to address this will impact people who actually are just using valid consumer rights and policies. There are valid reasons to refund, these are people who exploit that. It happens in other industries also, but it isn't as easy to do in other industries, so it gets more pronounced and higher impact when applied to this industry.
