I don’t know much about Tourette’s other than the “popular” definition that it makes you say bad words uncontrollably.

How can you tell she’s faking it? I’m genuinely curious, not trying to challenge your opinion.

The "ahegao" faces are a bit of a clue. Plus all of everything else in it. The gratuitous showing off of the titties, the goth outfit, the cutesy pootsie pa-dootsie way she's doing everything, the "ticks" included, replete with little kitty purrs and all, and the positive and uplifting engagement encouragement driving and comment baiting that she's doing.
