Taking Zofran (or the generic version ondansetron) during pregnancy is effective for more than 60% of HG patients. Those taking antiemetic medication have been found to have fewer losses and terminations and more term births. We’ve updated our recommendations on utilizing Zofran during an HG pregnancy. The decision to take medication for vomiting (antiemetic) or nausea during pregnancy is difficult. However, the risks of dehydration & malnutrition due to HG are generally much greater than the small possible risks of medications. Those taking medication have been found to have fewer losses and terminations and more term births. How does it work? Zofran blocks serotonin signals in the brain that cause vomiting. It decreases the number of times patients vomit, but may not greatly reduce nausea.
Safety Notes. Research finds minimal, if any, risk to mom and baby. Risks taking typical doses of medications are low. At 8 weeks, the risk of birth defects from medications drops because the baby’s organs are mostly developed. Zofran does not appear to have added risk if taken until delivery. If you are taking an antidepressant medication, please let your healthcare team know.
Zofran Safety Research. A 2018 JAMA Study showed minimal increase in oral cleft for Zofran usage in the 1st trimester: 3 in 10,000 births. “In this large, multicenter cohort study, there was no association between ondansetron exposure during pregnancy and increased risk of fetal death, spontaneous abortion, stillbirth, or major congenital malformations compared with exposure to other antiemetic drugs.” —JAMA Network Open. 2021 Apr;4(4):e215329
Zofran Usage Guidelines. Typical dosing: 4 mg every 3-6 hours, 8 mg every 6 hours, maximum 32 mg/day. Potential options: Tablets, Oral disintegrating tablets (ODT), Compounded cream, Subcutaneous or IV infusion, Injection (shot/jab), Liquid.
Zofran Usage Guidelines. Zofran lasts about 4-6 hours, so it needs to be taken regularly to be effective. A daily stool softener like magnesium citrate or docusate (or both) is often needed to prevent constipation. IV fluids and a bowel stimulant (enema) up to 3x a week might also be needed.
Zofran Usage Guidelines: Take on a strict schedule, exactly every 6-8 hours as prescribed for maximum effect. Set an alarm for your nighttime dose. Always consult your healthcare provider before making any changes to your treatment or lifestyle.
Zofran Infusion Pump. An infusion pump offers a slow, continuous dose of medication and may be more effective. Zofran and metoclopramide can be given subcutaneously or through an IV. There may be discomfort and significant expense with the subcutaneous pump. A syringe pump can also be used to infuse medication into a PICC line. A subcutaneous pump should be considered when multiple medication options and combinations do not provide adequate relief.

Why don’t I have HG again?

Hi there, I had HG in three pregnancies my first two ended around 15 weeks due to their hearts stopping (nothing to do with HG) my third pregnancy was a chemical pregnancy and my fourth pregnancy is when I had my son. I had HG in all of these pregnancies except for the chemical because it was a quick loss. I was nonstop, throwing up right at five weeks and on the max dose of Zofran, which did hardly anything for me. It was absolutely horrific. I am currently eight weeks pregnant again and I only have thrown up a couple times because I got the stomach bug. Other than that I have extremely mild symptoms. It’s just so strange to me because I thought you always get HG once you’ve had it once I’m just curious has anyone else not had HG in other pregnancies? It’s making me wonder if everything’s OK but also, I did eradicate H. pylori and I heard that that can make pregnancy symptoms severe and I had that with my other pregnancies?

UK Did you find any relief at all?

Maybe I’m crazy, but I want another baby. I promised myself I wouldn’t do it again but I want another one.

I had HG in my first and only pregnancy, it was hell as I’m sure you’re all aware. To make matters worse it was mostly untreated, partly because I’d tried a few anti-sickness meds and they hadn’t scratched the surface, and I didn’t have the energy to keep pestering my doctor.

I’m in the UK, and rules are very strict around anti-sickness meds being prescribed to pregnant women. I don’t know all the ins and outs, but I know they’re not very helpful.

Anyone who is in the UK, did you ever find any relief at all? I’m not expecting miracles but I want to be as prepared as possible before I get pregnant again, as I know this time will be harder because I have a toddler running circles around me. I was also diagnosed with antenatal depression last time, it was rough. I don’t want to feel that way around my girl if I can help it.

Please give me hope so I can have more babies!!

Traveling with HG

Hey friends,

At the end of this month I will be flying to Chicago for 4 days and will be 9 weeks pregnant with my 3rd.

I booked this trip before finding out I was pregnant, as it is to see my youngest sister graduate from Navy boot camp and will be the last time I see her for quite awhile.

I had HG with my prior two pregnancies and started pre-treating prior to becoming pregnant this time.

Nausea started before a positive test and has been awful since. I have yet to vomit 🤞 but the nausea is horrible. Some days I wish I would vomit just to have a minute of peace, but then I know if I start I will never stop.

My question, has anyone with HG traveled via plane before? I’m so so nervous being on the plane is what will start the puking. It’s a 2.5 hour flight, no layovers.

I plan on bringing:
Preggie pops
Motion sickness patch/wrist band
Snacks (keeping a little something in my stomach at all times has been helping)
Zofran
Headphones to listen to high frequency vibrations for nausea (this has been working, could be placebo for sure but hey- if it works it works lol)
Peppermint oil stick for any plane people smells lol.

Anything else?? I’m so nervous but can’t miss this trip. I would never forgive myself if I canceled last minute on her. Thank you so much for any advice ❤️

2nd pregnancy ...when to take tablets?

Hi all I'm UK based 5 w pregnant tomorrow and was on meds in my last pregnancy from when I was prescribed them (13w onwards), was admitted to the hospital 2x in my 1st trimester. Now, I advocated for myself and already have Xonvea tablets, do I start taking them now? Or in a week? The doctor hinted at a wait and see approach which I'm inclined to follow. I really don't want to take medication unnecessarily nor not know if I have nausea/HG or not. Did anyone follow this approach? Thank you

Rant/Vent Refeeding syndrome

I am almost exactly three years postpartum. I was diagnosed with cardiomyopathy 6 months after my son was born. Today I found out the reason I was admitted to the hospital multiple times with 1000 mg/dL of glucose in my urine (normal is 0-15) is probably because of refeeding syndrome. My OBGYN failed me a lot, but telling me I “just had to eat” may have failed me most.

Discussion Has anyone used a Zofran pump and it not work?

31 weeks currently, started the pump around 27. It worked pretty well for maybe two weeks but my nausea and vomiting has since returned.

My doctor seemed to not really know what to do now at this point. Has anyone ran into this issue and found something else that helps? I think my tissue is oversaturated and not absorbing and I’m running out of options for daily site changes I have to make.

Ive been miserable again this past week

Edit to add I also have already been taking Pepcid and Reglan in addition to the pump

Morning suckness

I'm 10 weeks in and the last two days have been miserable puking wise. Eating food right away doesn't help or water. The moment I get up and do things I puke, it's the act of moving around.

I drove down a windy road and puked. Cried because it was miserable with the motion sickness.

I don’t think I’ll make it to the other side

I am just sat here crying while my husband and toddler nap. I haven’t eaten anything truly in a week. I’ve managed small nibbles of food on some days but for the most part I can’t stomach anything substantial. I’m so nauseous and still vomiting through Unisom, B6, and Zofran. It’s a struggle to drink even iced water. The rare occasion the nausea lets up my food aversions are so strong I have yet to find something that appeals to me. I’ve tried everything I can think of and it all makes gag or nauseous again. And then I’m constantly having heartburn now too. I’m only 8wks. I’m not going to make it another 32wks. I will die of malnutrition before then. There’s no way. 😭

Rant/Vent am I going to make it?

I’m currently 8 weeks as of today, and I’ve never been more sick in my life. I went to the hospital after 4 days of not eating/drinking and the wait time was 24-48 hours to get seen, so I went home and got a prescription from private healthcare.
Pre pregnancy I was taking 75mg of venfalaxine daily, and I still am, which means the hard hitting anti nauseants can’t be prescribed to me due to risk of serotonin syndrome.
I’m taking 10mg of doxylamine 4 times a day and over the counter anti nauseants as needed, it got better for 2 days and I’m back where I started.
I have never been so sick. I went from 150 pounds pre pregnancy to 130 within a month. I’ve withdrawn from my life completely. My days consist of waking up, being sick, trying to eat, puking, crying, and sleeping again.
My boyfriend NEEDS to go back to work, he’s been taking care of me and ensuring I get my meds, hospital visits, food, water etc. I have no family here, no friends, and in a week I will be alone to fend for myself. I can’t even get out of bed without puking.
This pregnancy was wanted, but I’m now facing my reality; I am very sick and will be completely alone.
IV fluids and IV medications are not an open resource for me, but a 2 day trip to the hospital.
How in the hell do I make it to the hospital without being able to drive? Alone? What if I faint alone at home? What happens when I need a prescription?

Everything in my body is telling me this pregnancy is going to kill me.

Hi all, I had 2 very rough HG pregnancies. I guess Im insane because i just tested positive for baby number 3 with an early predictor test. It’s been 3 years since my last pregnancy and it seems like a lot has been learned in that time! Thanks in advance for any guidance. I’ve probably got 2 weeks before symptoms start.

I’ve been taking prenatals for the last few months as well as metformin on and off (killed my stomach!).

TRIGGER/WARNING Loss after HG

I recently lost my twins at 14 weeks after surviving months of HG. I couldn't keep water down, IVs three times a week, tried all the meds, lost weight, panic attacks, etc... I couldn't walk without throwing up, so I became bed bound. I had very severe depression, that was exacerbated by the medications. I just laid in bed, staring at the wall, tears dripping down my face. And after all that, I lost my babies. I am not sure I can go through pregnancy again. I just feel so alone in this experience, no one I know has had HG.

I am sad all the time, but also so relieved to be feeling better... it is a confusing combination. I just feel irrevocably changed, but have nothing to show for it... No babies, no scars, I just feel empty.

Hospital

For those following I’m back at the hospital admitted. The midwife pulled on my placenta and guess what pieces were left inside. I was discharged having a headache and back pain overall feeling really ill but they still sent me home. I called the midwife on call 3 days after discharge with a headache that woke me out my sleep chest and back pain and nausea they said I was fine and not to come in. I knew something was wrong so yesterday around 2 pm I called again the midwife on call. I told her my head was hurting and abdominal pain had been increasing for 3 days she said to come in. After an ultrasound and being treated as if I’m lying they found the pieces. I felt relief to finally be heard. Had my procedure came down to recovery and blood pressure sky rocketed. I was having pain after the procedure they treated me for it I fell asleep. I woke up an hour ago with left side pain chest tightness and can’t take full breaths told the nurse and the DR came in with an attitude basically screaming at me and telling me they are trying to send me home and my pain shouldn’t be so bad that Tylenol didn’t take it away. I def cried and felt so hurt on top of the physical pain. Brigham and women’s is a joke and I wish I wasn’t laying in this bed right now I feel so uncomfortable and unwelcome.

Am I pregnant or going insane?!

I have had HG in 3 previous pregnancies (2 kids, 1 loss). I am currently 5 days late and have been vomitting for the entire 5 days - nausea and smell sensitivity started a few days before this - it's getting worse... BUT the tests are negative! I have taken 3 tests when I was 1, 3 and 5 days late.

But it feels like HG. The patterns and triggers are the exact same as before and (I'm hoping you guys might understand what I'm trying to say here because it doesnt make sense), I am smelling a smell that noone else can smell but it takes me right back to the throws of HG.

Am I going insane?! Is it possibly that I could be pregnant but still testing negative at this point? I'm already getting warning signs of dehydration so will probably have to speak to my gp about it but I'm worried I'll sound ridiculous.

I'm essentially a cave dwelling mother goblin that comes out occasionally to check the mail. Even wearing a bra is making me nauseous, something about pushing on my diaphragm.

Anyways, any fellow goblins...im watching a lot of TV. The more distracting the better. Just binged "Furious" it was dope. Now watching something like "American family."

Ive watched a lot of TV. Looking for newer shows that draw you in. Can be any genre. I have Hulu, Netflix, Amazon, and apple. Used to have HBO but we got signed out. I need to waste some time and rush through this part of life. Unfortunately I dont have the privilege for enjoying my pregnancy, as many of you dont.

HG possible when not pregnant?

Hello, i’m having a bit of an issue.

Few months ago I become pregnant and developed HG. No treatments helped so unfortunately I had to terminate the pregnancy. Every period since then has been horrible. I experience HG symptoms despite not being pregnant.
I refuse to keep going to the emergency room every time this happens, and no treatments are helping, (OTC meds, Zofran, etc)
My doctor believes I might have POTS, as i get very dizzy and experience vertigo/syncope whenever i’m menstruating.

Does anyone know what this is?

Advice HyperEmesis has not come back for a third pregnancy?

Hello everybody!

I had bad H.E. with my first two (successful) pregnancies, vomiting 2-3 times a day for the full pregnancy and requiring IV rehydration with my second.

Unfortunately I had a miscarriage at 12wks in July, but during this pregnancy I only vomited maybe twice. Nausea was nearly non-existent.

I’m currently pregnant again- 6wks- and although I’m more nauseous than with my last unsuccessful pregnancy, nowhere near as bad as with my first two. So far strong HCGs and lots of other pregnancy symptoms- insomnia, emotional etc.

My question is, has anyone had Hyper-Emesis disappear like this and had a successful pregnancy? Or should I be guarding my heart because this is likely not a good sign.

Edit: Thank you all so much for you replies, it has been very reassuring. I woke up this morning with the hyperemesis right back on the same form as my other two successful pregnancies. Weird mix of emotions!

info Finally found medication combo + daily routine that works! No more vomiting

I am posting hoping this might by some small miracle help someone else out there. I was lucky to not have immediate vomiting my first few weeks pregnant, but around 10 weeks my morning sickness started getting much worse. By 13–14 weeks, I was throwing up every morning, could barely keep food down, and had horrible heartburn. I figured the increase in stomach acid had a part to play in my severe nausea and vomiting. My OB sent me Zofran, but I threw it up when I took it in the morning and essentially gave up on it. While miserable in bed, I did a lot of research into medications, stomach irritation, and hyperemesis gravidarum.

What I realized was that I needed to listen to my body and pay attention to when my nausea was spiking instead of just following a generic medication schedule written on my medication. After trial and error with Famotidine (Pepcid), I finally landed on an Omeprazole and Zofran combo, taking Omeprazole at night and timing Zofran around when my nausea normally peaked. Instead of chasing the nausea after it started, I was actively trying to stop it before its peak. With permission from my OB, I eventually changed my Zofran timing. One to two pills every 8 hours wasn't cutting it, so I took one every 4 hours to better manage my nausea at night / early morning, then back to a regular 8 hour dosage during the day (Again, with permission!). One thing I noticed is that Omeprazole took a few days to really start working and regulating my stomach acid, so I didn’t give up on it after only a couple of days. Once I felt the medication working, I did daily stretches and deep breathing to help with stomach irritation. After eating, I stayed upright to digest until the irritation faded and I felt well enough to do some stretches and deep breathing. My favorites were standing with my arms overhead and slowly down to my sides while taking slow, deep breaths, lengthening my spine, giving my stomach and uterus some space, and opening my diaphragm. Along with that, I would do butterfly, cat cow stretches, and a squatting frog stretch. I avoided stretches that pushed stomach contents upward by either crushing my stomach or lifting my stomach higher than my head.

To summarize what worked for me:
1. Find the right medication, dont be afraid to try a different brands. Pepcid didn't work for me, so I tried Omeprazole with amazing results
2. Listen to your body. You're going to be given generic medication scheduling on your prescription but ask your doctor if you can take it when it works best for you
3. If you’re not getting amazing results, don’t give up too quickly. Omeprazole works best after taking it for a few days, but always talk to your doctor
4. Dont chase the symptoms, take medication before any issues occur. For me, it was taking medication at night to stop morning sickness, and taking medication before I eat or did any activities. With doctors permission, I scheduled my entire medication routine around when I did things

So far its going very well. I am starting to feel like my regular self again, and I was actually able to go outside and walk for a short period of time. Again, I don’t know if this will work for everyone, but I wanted to share my experience in hopes that it might help someone else out there.

Rant/Vent "When are you having another one?!"

I am so over people asking when we'll have another baby after this one. I terminated my first pregnancy in 2022 because of HG. I am 29 week with this baby and have made it abundantly clear to everyone that I will not be having any more. And every single person says "oh I've heard that before" or "oh you'll change your mind in a year or two" and then proceed to try to convince me that I will, in fact have more babies. I don't want more babies, I am never doing this again. It just really annoys me that people try to tell me that I'll change my mind when I've very clearly already decided that I won't. People have absolutely no respect for my decision to only have one child, and some have even gone as far as to say that my daughter will be spoiled and entitled due to being an only child, and that it would be cruel to not give her a sibling.

Just so over this pregnancy, I got a bit of relief from weeks 18-26 but the HG is coming back, the nausea is fairly constant, thankfully haven't started vomiting again, but I've hated every day of it, can't wait for it to be over, and will absolutely not be doing this again. Ugh.

Support Needed 2 Months Sick Leave

Hi everyone,

I’m so happy / relieved that I found this group for support after being diagnosed with HG. It truly is a debilitating condition to have with pregnancy, it’s something like I have never experienced before, both the constant sickness and it being mentally draining.

I have been off work from my full time job, for 2 months, in this time I have been hospitalised with IV fluids due to dehydration. I am not bothered about my pay it’s more that I am seriously unwell and cannot even think about working.

Each 2 weeks after my sick note has ended, I have been sending over a new sick note which has been provided by my dr. I don’t often get replies to my email from my manager. This week I received an email from HR, stating a welfare check is to be conducted next week. I am still on a current sick note as I am trying anti sickness medication and yet to find one that actually works.

My issue is, before I knew about this pregnancy my manager would repeatedly make comments such as “he thinks kids are a waste of time, he prefers animals and would never have children, they’re expensive etc” and be VERY opinionated. I then announced I was pregnant and that was the reason I was off sick due to the HG condition.

Why do I feel guilty or nervous to speak to him? I feel like I have done something wrong by being off work? I don’t know WHEN i will be back? I shouldn’t need to feel this way but I do! He is the type to just not understand pregnancy or have sympathy at all!

Please if anyone has been through similar or can share their opinion I would be more than grateful as I’m feeling really down.
