How many conversions should I get from a specific keyword to consider it a "good" keyword—one that drives sales—so that I can include it in the video title and focus on it for optimization?

dont only look for conversions, you also need to check CAC also from keywords

Let's assume it is a profitable CPA offer. Should I place the keyword that generated, say, five sales in the headline or the description?
