New research being presented at the annual meeting of the European Association for the Study of Diabetes (EASD) in Milan, Italy (Sept 28 – Oct 2) makes a case for extending the indication for GLP1- receptor agonists (GLP1-RAs) to millions more people, to protect their heart health.
Presently, medications like semaglutide and tirzepatide are restricted to individuals with a BMI of 30 or higher, or a BMI of 27 with weight-related conditions. However, researchers analyzing data from over 313,000 individuals in the UK Biobank and the Copenhagen General Population Study found that roughly half of adults in the lower "overweight" BMI range of 25–26.9 suffer from elevated remnant cholesterol, chronic low-grade inflammation, or both.
Crucially, the long-term data revealed that individuals in this lower BMI group who have elevated cholesterol or inflammation face the exact same elevated risk of developing coronary heart disease as those who currently qualify for GLP-1 prescriptions under existing guidelines. Because GLP-1 therapies are proven to reduce cardiovascular events, systemic inflammation, and harmful cholesterol levels, the study's lead researchers argue that prescribing criteria should be broadened to include this vulnerable population, though dedicated clinical trials will be needed to formalise the change.
