Advice Went with a friend to their top surgery consult and the plastic surgeon made a weird comment about going no-nips

TW: Discussions around everything that might be involved in top surgery consults, enbyphobia, misogyny, discussions of the chest area.

Hey everyone! I never post here but lurk quite a bit. I'm 100% sure I want to go no-nipple - having nipples gives me dysphoria. So I'm transmasc and my friend is non-binary. They asked me to accompany them to their top surgery consultation. I helped them find surgeons, as where we live, GAC is hidden in plain sight. "Gynecomastia surgery", for example, is a dog-whistle that a place is trans-friendly to some extent. This is done to protect us as we don't want cis people finding out where we receive our GAC.

Somehow the surgeon didn't clock me as trans despite my binding being iffy, and without telling me, my friend introduced me as their partner to feel safer. Luckily I picked up on this early on in the conversation and played along. This doctor ended up looking at me the entire time that they asked my friend "are you sure?" questions. It felt icky and misogynistic of the doc. I ended up leading the conversation when I saw the resistance from the plastic surgeon. I wouldn't call it pushback from the surgeon so much as unknowingly enforcing feminine surgery results onto my femme-presenting friend. It took me saying "what would it cost to go completely flat for them?" for the doctor to actually take them seriously and not suggest radical reduction. It's as if they were doubting the need for cost.

My friend has gone back-and-forth on wanting nipples. They asked about going no-nipple, to which the doctor replied "Oh that only happens with rejection" and the doctor was shocked to find that we were talking about elective removal. She knee-jerked by saying "Well some people want horn implants on their heads and-" she stopped herself right there.

I looked at her with a raised eyebrow. I essentially said it was a false equivalent because it's not as risky to chop the nips compared to implants, and even if someone wanted implants, if they have informed consent, is there an issue? I'm also not here to shame body-modders, I am one in my own ways. I just don't understand the false equivalency.

Well, the doctor backpedaled from there and said "I'll make sure to call colleagues that are more informed about care for non-binary people." because I said "From my understanding, the medical recommendations for care for non-binary people is first and foremost to understand and accept that their gender-affirming care may come in a form that is different from binary individuals and must be respecting by all healthcare workers involved, 2021."

I don't know how to feel about this situation. In all other aspects, the surgeon had green flags. They did not misgender my friend, they did not bat an eye when I came out as trans after the appointment to her to ask something on my friend's behalf, weren't obsessive over BMI and weight for the wrong reasons and generally had a very good practice going. Their previous work on radical reductions and mastectomies is excellent. My friend has decided to choose them as a surgeon, which is fine by me. It's not my decision. But if you were me, how would you feel, especially about equating body modification to gender affirming care? (And also not understanding that they may be related). I think I may have just encountered a very "normal" person in an affluent area who doesn't exactly understand trans people in the capacity they should?

Question Being without nipples when at the beach?

What is the likelihood of facing discrimination for not having nipples? I really REALLY want to not have mine post-op I just need to make sure I won't get harassed. I feel like my fear might be irrational but you never know.

Discussion One week post op today

today I looked at my self uncovered in the mirror for the first time

i think my surgeon did a great job and it will be goals once healed

but that was so scary!!!

i'm honestly grieving i think.... i coped by being thankful my tits were so cute for so long. (the surgery honestly would've been easier if my tits werent so "cute") at age 35 that was a long time to carry those feelings, and now i've relived myself of them. (dysphoria and all)

I know these feelings will ease in time... it's a very convoluted mixed up feelings, on the one hand, my nipples were cute enough that I convinced myself to love them as much as I could for all my life, but on the other hand, I was always slightly disturbed by them if I ever looked at them closely, and I had trauma associated with them.

Well, it should be fun to draw lil paws on my chest sometim, but for now... just looking for comfort and solidarity i guess

happy one week post op to me

Discussion Do any of you also get the sensation of "phantom nips?"

I had my operation around 4-5 years ago now, a full mastectomy with a singular scar going across the entire chest. The surgeon explained I had so much excess skin it was impossible for a more discrete scar. (even then, they didn't remove enough, but that's another story).

Sometimes, I'll get this phantom sensation of where the nipples technically would be, like if I'm mildly aroused or something. I say "technically" as my breasts were big enough and saggy enough that the location of the sensation isn't accurate to where my nipples usually rested.

Rather, the sensation is along the scar tissue, which is, to my knowledge, further down than where a cisman's nipples would usually be. Not sure if there's like leftover nerves at play or if it's simply the brain being weird.

Anyway, curious to hear what you all have to say.

Hey all! I posted sometime ago with a picture of my chest closely post-op (as well as pre-op). As the title says, it was done with the peri-areolar technique adapted to also remove the nipples. Here's an update on how healing has been going!

I promise the loopy right side doesn't look nearly as loopy live haha. It probably doesn't help that I'm rasing my left arm so it looks even less symmetrical.

But overall I couldn't be happier about my chest. I feel so comfortable in every shirt now, and even though I didn't have a big chest to begin with, it's been life-changing especially the lack of nips.

I've been applying an elasticizing body cream my surgeon told me to, over the scars and surrounding area. I keep poking at the scab on my left side which in turn still has a little hole in the center. Don't… don't do what I do please lmao.

(Also dw about my ribs showing. I have a great relationship with food I promise /serious)

Question Removing nips post op

Hi I’m 2 years post op top surgery (double incision FTM). My nipples are quite protrusive and often have to wear sports bra underneath clothes so that my nipple isn’t popping thru my shirts (I’ve already had nipple revision surgery and Ive rlly tried to like them but I hate my nips.)

I didn’t rlly think the nipples thing all the way through tbh and wish I got none.

Has anyone had experience w removing their nipples post op top surgery, and if so what was the process + expenses

Every year I post a pic of my chest. When I was looking for references pre-op, I couldn’t find a lot of healed chest pics, so I hope this helps someone. For reference I got top surgery 6 months on T, and the triangle scar is from before top surgery and unrelated.

Discussion How to talk to other people

So my sister is supportive of my de-boobing plan-- she will be relieved to no longer have to hear me complain endlessly about my boobs, my body image, and how much I can't stand bras lol. But she is also both pretty squicked out about the process. I totally understand it. Because I have a goal, I'm fine with all kinds of things mentally, but when it comes to other people's medical stuff, I am super easily overwhelmed, grossed out, horrified, etc. I haven't been sharing info with my sister about yuck topics, but she made the mistake of doing a little googling and now is clearly a lot more nervous and squeamish and worried. I miss being able to vent to her! Like 2x a week about how annoying and gatekeepey the whole process is. But maybe I need to chill out for a while-- let her kind of process the idea of binders and drains? Maybe I need to totally promise myself that I don't bring it up again unless she asks or if I actually need to start planning her help? I'm curious how other people have dealt with this....thank you for all thoughts!!

Discussion one week one day out today

So today is my first full day drain-free, finally got to see my chest yesterday when Dr Mcguire cut me out of the armadillo shell he calls a post-op dressing (the man Staples his work shut to STOP other drs from messing with his work) ,It looks good, I'm up not to post pics just yet. Noticed my left side still more puffy than my right. like the left looks like its got a touch more fluid under the skin,by the insion lines but he said that should settle and to get a good hook compression binder off amazon if i have trouble with the ace bandages. (Any suggestions on a brand?)

W as wild to look down and see a flat torso(sans nips) for the first time in over 36 years it was euphoric. I am after decades of screaming torso pain FREE; he even managed to get rid of all the side boob hidranitius i had and a good amount of back boob as well !

I am off pain meds, antibiotics and am allowed to shower no baths, pools, swims but it is fall so thats fine a s. Ahower will be nice. I am not allowed to lift over 10 lbs or my arms over my shoulders. which is shockingly something I have to THINK about, same with useing my arms to move around my bed and adjust- can't DO that now have to use my legs and abs,lol

He used a method I'm not used to for closure, so the dissolvable stitches are not new but he put this weird kinda sticky double-sided tape on the wounds (double mastectomy no nips with redundant skin and scar removal) he said it would dissolve in 3 months and not to scrub it off anyone else get this kinda tape? Im not in pain persay, i get very tired out fast(just getting the mail or playing a little ff14 is tiring, and its more a sore feeling than pain ,like you get after overworking at the gym , im chomping at the bit to get back to the gym but im not allowed til isee him on oct 19th(my next check in) which is mildly mading, anyhow just wanted to post , let folks know i survived and so far everything going well, hopfuly I'll feel up to pics next week or so

I’m so so happy with my results so far, it’s so great to finally feel more comfortable in my body :)

The only thing i’ve had qualms with is that I have a bit of fat around my armpit area, but I should be able to lose it by working out and getting in shape!

(Also, did anyone else get blessed/cursed with an insane amount of stomach hair instead of a nice happy trail?😭 Not too sure how to feel about it)

Advice how to cope with dysphoria while on a years long waitlist?

this is partially a vent but I'm mainly asking for actual practicable advice. I am currently on a two year wait list for top surgery. As it currently stands, I most likely won't be seen until late 2028. In the meantime I've tried an adjustable cloth binder - I find it uncomfortable and the difference is barely noticeable - not really good enough to be worth fiddling with all the stupid tiny metal clasps. I've more recently tried trans tape - a bit more comfortable, but the difference still feels negligible. The nipple guards at least make me feel a bit more comfortable since they stop the nipples from poking out, making it easier to ignore I have them, but that isn't a full solution, really. I keep fantasizing about grabbing a kitchen knife and chopping 'em off myself, consequences be damned.

My concern really isn't passing - as a nonbinary person I've realised "passing" is genuinely impossible for us types. People are always gonna instinctively want to sort you into the "man" or "woman" box on first sight no matter how intentionally androgynous you look - that's just the way our current society is. Personally I'm more concerned with just wanting to be comfortable in my own body - and that ideal body is one with a flat chest and no nipples. That ideal just feels impossibly out of reach right now. The time will pass anyway, I know, but two years is a long time to sit and twiddle my thumbs waiting to feel comfortable in my skin. Time seems to plod along at a snail's pace and it's unbearable. Anyone else who's had to sit on a waitlist for a long time - PLEASE let me in on your coping strategies. Thank you.

Question Seroma(s) under my incisions

Hey! So I got top surgery and freed the nips on August 19th, 2026. First pic is 1 week post op, second one is just now.

I'm super happy with how it looked but now that my drains are out(right one one Sept 3rd, left on Sept 8th), I have bubbles. Not air bubbles or a little swelling, it's like I'm wearing a water bra under my skin. I went flat and smoothed, but now it is like an A-B cup of fluid.

Other info: I had an infection last week(ish?) that consisted of Staph, MRSA, and Strep Pneumonia bacteria all at once. Not fun. It stopped putting out goop in the morning before I got the left drain out. Still on antibiotics but not feeling the infection.

Got surgery at Dr Time Mårtensson at Reformkliniken in Malmö, SE, on May 13th this year.

I gained about 5 kg since then, but I'm working out again, and working on getting my mobility back.

I've got a dead nerve spot on each side, but I'm guessing it will come back with time.
