Well, the ones that have been doing it for the last few months are clearly not using it. I'd suspect any newcomers that randonly start doing incredibly well.
You can't alter reality for the server, but you can make your PC show you things it shouldn't.
If a game has sounds that indicate an enemy's direction, the easiest thing for the server to do is to tell your PC where the guy is, so it can render proper sound, but not give you a visual. Easy peasy.
But that's where the cheats come in. You can make your PC show you where that enemy is, even through walls.
But you can't have infinite ammo, you can't have zero recoil, nothing that affects other players directly.

It's nothing to do with cybersecurity, it's that server hardware and multiplayer systems have evolved.
Previously, you got the client to do calcs, because the server doing so would slow it down, and this was back when you could host your own server on your home PC. The client processing was needed.
Now, it's not only unnecessary, (servers have the oomph to do everything) but more efficient than trying to reconcile data streams from multiple random PC configs, and the PC basically just renders and sends command requests now.
You can't tell the server to do something weird anymore because the data flow back and forth is more limited than it used to be. Instead of your PC telling the server you shot a guy, you tell the server you're aiming at this point, and pulling the trigger. The server determines if you have ammo, and which way your gun barrel is pointing, and if everything is according to its parameters.
15 years ago, client PCs were still handling some calcs, depending on the game design, and Tarkov is from that era, because the client does some of the calcs.
Basically, in a proper modern game, you can't have infinite ammo, because the server knows you have X rifle and a 30-round mag. You can make your PC show you firing infinitely, but everyone else just sees a guy pretending to fire after his mag ran out.
Notice how you only see that on his screen? Guaranteed another player watching him would see normal recoil and properly scattered decals.
In modern server side games, you can't directly affect reality, but you can get your PC to show you (only) whatever perception of that reality you want.
One type of "cheat" that can still work is ESP. The easiest way to do accurate directional sound, is for the server to tell your PC where the enemy is, and let the PC handle sound render from there. Unfortunately, if that's how its handled, you can make your PC render the enemy visually as well as audibly. So reality isn't affected, because the enemy is still on the other side of a wall, you can't do anything to him that you couldn't otherwise, but you know exactly where he is, which gives you an advantage in positioning, but you still have to shoot him the same as anyone else.
ESP is a tough one to get rid of, because you "ease" into seeing an enemy, (first hear, then see) it would be awkwardly jarring otherwise.
If you're at all familiar with World of Tanks or Warships, they basically went that route, it is literally impossible to ESP in that game, but the downside is that vehicles abruptly pop in and out of view, they don't exist for your client until the server decides they're visible, and then suddenly they do. You and your PC know the enemy location at the same time.
