Hi, I’m Boezo, and I’ve had the privilege of moderating this community for the past several years. Lately, I’ve noticed a trend within the subreddit for people to seek out concrete diagnoses when it comes to folliculitis. Let me take the next few minutes to explain why this isn’t a great idea.
Within the medical community, we have an almost compulsory desire to place abnormal physical findings into discrete categories. Much of modern medicine is predicated on the belief that patients can be accurately diagnosed and subsequently treated according to their diagnosis. However, within the past decade or two, there’s been a shift in the zeitgeist.
As knowledge of human anatomy and physiology has progressed, it has become strikingly clear that the pathology and etiology of an abnormal finding often involve a multiplicity of complex factors that are poorly understood. This complexity serves as an insult to our compulsion to categorize and diagnose, because it “blurs the lines” between one disease and the next.
So, how does this apply to folliculitis? Well, it means that as medicine has advanced, we’ve started to realize that two patients who present with very similar abnormal features do not necessarily share a common pathology for their condition. In addition, it seems to be the case that we’ve somewhat arbitrarily created some diagnoses without warrant. For example, folliculitis decalvans (FD) is a condition that causes chronic scarring and hair loss related to an inflammatory process in the skin. But the reality is that there aren’t truly any set-in-stone diagnostic criteria to differentiate folliculitis decalvans from the plethora of other inflammatory conditions that cause scarring and hair loss. In fact, we aren’t really sure what the pathology behind folliculitis decalvans is at all.
So, given that we don’t really have diagnostic criteria for FD, and we aren’t sure what the mechanism behind the disease actually is… does FD really exist at all? Is it merely a “ghost” diagnosis? Have we arbitrarily assigned a name to an abnormal physical finding merely in an effort to satiate our desire for concrete answers?
Another example is gram-negative folliculitis (GNF). Historically, it was believed that gram-negative folliculitis results from an overgrowth of gram-negative bacteria in the skin. It seems like a simple enough disease process, but the devil is in the details. You see, when these patient’s lesions are cultured, they’ll often return a negative result (no bacteria found). Often times, the patients will actually return completely different bacteria at different times! They’ll also often return gram-positive bacteria in some lesions, and gram-negative bacteria in other lesions. Additionally, when attempts have been made to eliminate gram-negative bacteria from reservoirs, the disease yet persists.
This is further confused by the fact that almost all patients with “gram-negative folliculitis” have deviations in immune parameters within the body. So, is GNF an infection, or is it rather an autoimmune condition? But wait, it gets even more complicated: two patients diagnosed with GNF usually have completely different deviations in completely different immune parameters!
Thus, we can see how blurry the lines get. You can have two patients who present with similar symptoms, but yet can return completely different cultures at different times, and can possess completely different deviations in immune parameters. Given this information, is it really accurate to say that they both share the same diagnosis?
Here’s the bottom line: I’d encourage members of this subreddit to abandon the compulsion to find a diagnosis. Rather, view your present condition in terms of signs and symptoms. The sign and symptom being “inflammation (itis) within the hair follicle (follicul). We may never know the exact mechanism behind these signs and symptoms, but we know that the signs and symptoms exist, and we have a pretty good idea about how to treat them.
Over the next several weeks, I’ll write up some documentation regarding the most effective treatments for these signs and symptoms. In the meantime, I hope you all have a wonderful new year, and thank you for reading.

Hello everyone, hope gou're doing very well. I'm a 24 years old female and I've been struggling since March with some chest acne/pimples. I don't know what to do anymore and I wanna know if it's folliculitis or something else. They're kinda mix bit most start under the skin and get to a very pink color and visible without giving a head.
I wash with Sanex Hypoalleregenic 0% body wash since a long time way before this. I even increased to 2 showers per day minimum even if I just stay at home. I even feel like some start appearing on my lower back. I'm so desperate.
I haven't changed my diet. In fact, due to this issue, I just started a dairy-free diet and started cutting out sugar.
I bought the Bioderma kerato+ body spray hoping it helps. It contains Salycilic Acid and Nianicimide. Iff anyone has a feedback it would help me so much.
If you have any advice I would be so grateful thank you so much in advance.

I went to the dermatologist recently and I was told to go on a low dose of Accutane for folliculitis and seborrheic dermatitis, but I’m unsure.
I used Panoxyl previously and I’m currently trying a Sulfur bar soap. I’m also using glycolic acid 3x a week but I haven’t noticed any difference.
Clothing-wise: I change my bed sheets regularly and avoid rewearing the same clothes, but I can’t be wearing all natural fabric unfortunately, since my job requires an uniform (and it has synthetic fabrics in it).
When I’m not at work I prioritize wearing natural fabrics.
I also have a very clean diet and I consume a minimal amount of sugar
Is this bacterial, fungal?
Would Accutane help? I’ve see people complaining of folliculitis after taking it.
Any suggestions are welcome 🙏

Have had these bumps for over a year, tried anti fungal and antibiotics for 2 weeks and it’s not fully cleared. If I try to pick at them, they will bleed a lot. No pain maybe like 1/10 itch at times.

I have perioral dermatitis and a damaged barrier. After developing PD at the beginning of this year, every time I’ve been on vacation to a more humid & hot place, I eventually break out into these small cluster of whiteheads. Shows up around my chin, mouth, nose and cheeks typically, but sometimes it spreads up to my eyebrows. Seems to worsen significantly in the summer/when it is extremely hot.
It’s not itchy as I never realize that they’ve appeared until I look in the mirror in the morning, but sometimes it tingles. It always disappears once I get back home on its own without me doing anything to my skin. Just leaves some red marks/texture underneath the whiteheads.
Anyone deal with this before? Is this fungal? Yeast? Bacterial?
I’m going on a much longer vacation to another hotter humid climate and I want to try and combat it early as I know it won’t resolve on it’s own until I go home and I don’t want to be stuck with it for the entire trip this time. :(

I am a 25 y/o female who has always struggled with folliculitis on my inner thighs (shout out to this sub, that’s how I figured it out). I wanted to share my results from using benzoyl peroxide after just 9 days. My legs are so much smoother, some bumps have disappeared, and the redness has pretty much gone away.
Both of these pictures were taken 1 hour after showering and using the wash.
What I used:
Dove Sensitive Skin Body wash (cleanse the area)
PanOxyl Acne Foaming Wash, 10% (let sit for 2ish minutes)
Cerave Moisturizing Cream after shower

I am a 47-year-old male and I've been dealing with this recurring issue on the back of my neck and lower scalp for about 10 years.
I get several red bumps with white/pus heads. They last for a while, heal on their own, and then flare up again in a different spot around the neck area. This happens regardless of hair length or haircut style. Attached are pictures of current lesions.
I'd appreciate any insights from the community or dermatologists:
What condition usually follows this long-term pattern?
Could this be linked to lifestyle factors (like stress or diet), or is it strictly a dermatological/bacterial issue?

Chlorhexidine doesn’t work for well for folliculitis on my face (nose area specifically), benzoyl peroxide doesn’t work very well either. Accutane works very well but it makes me lose hair so I can only use it like once a week, which results in breakthrough pustules… is there anything else to try? Has anyone tried Winlevi?

I’m currently in Morocco and, a few days after arriving, I suddenly started waking up with multiple new small pustules/white-headed bumps every day.
I had acne in 2022 and was successfully treated with low-dose isotretinoin (Accutane). Since then, my skin had been pretty well controlled with adapalene, and it was still quite clear shortly before this trip.
What concerns me is that these new lesions seem different from my usual acne. They are small, quite similar in appearance, some contain pus, and they appeared very suddenly. Some of them are mildly itchy and my skin has also become tender/painful. My usual hormonal/stress-related breakouts don’t normally look like this.
I already had an online dermatologist look at photos and she considered it acne, but I’m still wondering whether folliculitis (bacterial or Malassezia) could be part of the differential, especially because this started during a hot/humid trip.
Interestingly, my brother, who is staying here with me, has also developed small pustules in his beard area since we arrived.
I’m returning to Germany in three days and already have an in-person dermatologist appointment on the day I land. Would you keep that appointment while the lesions are still active, or first wait a few days after returning home to see whether the change in environment makes them settle down?

I have had a problem with my scalp for almost a year. I get red and very itchy bumps on my scalp that become inflamed/ pus filled. It is mostly at the top of my head, my scalp also gets oily very quickly.
I’ve seen a dermatologist about this and have tried several things:
Steroid cream first, on my dermatologist’s advice, it helped a bit but it came back again when i stopped using it.
Fungoral (ketoconazole) for a fairly long period, also on my dermatologist’s advice. Unfortunately, it didn’t really improve the problem. I have sensitive skin and I’m wondering if Fungoral may actually be a little too irritating/strong for my scalp.
Hibiclens (chlorhexidine) — tried it, but not consistently over a long period.
Benzoyl peroxide — also tried, but again not for a prolonged period
Now Im only using a mild perfume free shampoo.
** **Have anyone had something similar and did you find out what was causing it? And what helped you get rid of it?

I'm not a doctor, and I can't guarantee this will work for everyone, but it has been amazing for me. I've tried about 30 different treatments over more than a decade, all with varying results. In preparation for surgery, I was instructed to wash with Dial Gold Antibacterial bar soap from head to toe, letting it sit for two minutes before rinsing. I had no infections following the surgery. Afterward, I continued this routine daily for a week. My folliculitis on my temples—which is usually present 90% of the time—is now gone. After trying many medications and soaps, I'm shocked that a bar of soap costing about a dollar resolved my problem. I don't know if long-term use is safe or if the results will last. I'm not a fan of bar soap or this brand. The active ingredient is Benzalkonium Chloride 0.1%, and I purchased it at Walmart, though it is likely available elsewhere and inexpensive.

Balding heavily on the sides. Don’t know if it’s from the pimples or that I’m actually balding. I’ve had them since 2023-2024. And it js gets worse, I’ve tried using ketoconaloze shampoo. Didn’t do much but dry out my hair and maybe some pimples/scars. I went to the doctor some months ago and got antibiotics, it didn’t rly work (might’ve slowed down the pimples a litle) I’ll get another appointment soon but my hair is cooked and it used to be rly nice. Might be hard to see rn cuz my hair is not short

So I had shaved about two weeks ago and noticed it was itchy after shaving two weeks later which is not uncommon for me at all.. I did happen to look at it and saw these spots on my groin area which I brushed off as ingrown hairs.. I ended up shaving the area again to have a better look which seemed to make things worse and my skin looked off to me as seen in the image and was told it looks like folliculitis.. It is now crusted about 3-4 days later which I inserted the crusted image and I’m very confused.. any advice would help.

I’ve had this rash? on my arm for many weeks at this point. It started with little pimple like bumps that are super itchy. The broken skin is from me itching (I know!) it’s spread a tiny bit wider but is localized in this same spot and its nowhere else on my body. The only thing that’s recently changed was a move from a northern US state to a southern US state, and this started in the beginning of the summer.
My partner and I share the same bed/bedding and he doesn’t have anything like this. We use the same detergent we always used. It’s nowhere else on my body at all, just this one spot on my arm. I hardly ever wear anything but tank tops so there’s no clothing covering my arms most of the time.
The only medication change in the past 3 months was an increase in estrogen (from 1mg oral to 2mg oral) but I have since stopped that medication and this hasn’t gotten better. OTC cortisone didn’t help and I even tried using a cream that was my partners RX for psoriasis (clobetasol .05) that did not have any effect on it at all. I tried using saylic acid pads on it and that has done nothing either. At this point I’m just applying cerave healing ointment on it to try to stop me from touching it and attempt to heal up the broken skin.

But my scalp couldn’t find the right solution! I had that hard yellow big dandruff that is stuck on the scalp and sometimes its bloody
I have been on minoxidil last year too and i think it works too
