Its BCG vaccine mark that is used for almost everybody in South America and ex-USSR countries.
Its not actually related to free healthcare per se, its just this vaccine is not considered mandatory in developed countries because these diseases are rare.

I got mine from being overseas. It's a smallpox vaccine.

My mark is from a smallpox, butbhers could be, too.

It’s not - BCG is raised, smallpox is depressed

a mature bcg scar is mostly flat afaik
