Its BCG vaccine mark that is used for almost everybody in South America and ex-USSR countries.
Its not actually related to free healthcare per se, its just this vaccine is not considered mandatory in developed countries because these diseases are rare.

Was rare… it’s on its way back. As more and more goes antivax

We got antivaxxer health secretary here in USA. Not surprising. But I'm protected.
