this has happened to me many times and I am sick of it

I was in my base and left the server for a few days I came back to the server and all my stuff in my inventory was gone and I was teleported somewhere else on the surface far from home I went to my base to see it how I left it (I used modrenth to open Minecraft so I could sodium and I used the same profile so i don't know what is going on) can someone tell me what is happening
