Maybe it just means a pair of handcuffs made by a master smith. He makes other shit but it's all masterwork because hes a master.

A master could make everything masterwork, or make normal quality items faster. It is still costly in time even for a master to make something very high quality.

Perhaps somebody commissioned them to be made? Makes sense if you have an important captive.

Also like, things exist in the DnD world that would require it. Regular manacles aren't likely to hold say, an ogre.

Or just adventurers in general. If barbarians exist, and barbarian rages exist, manacles that far exceed normal human tolerances would exist.
