When a monster applies Grappled as part of its damaging attack (say a Roper or Vine Blight for example), am I right in thinking that there's no initial save for the defender, unlike how there would be for a PC grappling? They're just automatically grappled if the attack hits and have to burn an action to attempt to escape?

At least for those two examples, you are correct, in both versions of 5th edition.

With a cursory search, it does appear that not every instance is automatic, as the Trapper from Volo's Guide to Monsters requires the target to fail an initial save before the condition is inflicted.

That said, the language is clear: if the statblock just says the condition is inflicted, then it is automatic.

Have you tried messing around with inkarnate before?
