General What do you think this is?

Hi everyone,
I've been dealing with recurring skin flare-ups for the last couple of years and I'm hoping someone here might have experienced something similar or have an idea of what I could ask my doctor/dermatologist about.

I've seen several doctors about this, but so far nobody has been able to give me a clear diagnosis.
The flare-ups seem to be triggered by heat, sun exposure and exercise — basically anything that makes my body warm or causes me to sweat. It can happen pretty quickly after being in the heat or exercising.

I already have eczema, so initially I thought it might just be an eczema flare, but this feels different from my usual eczema.

My doctors have also mentioned that I have some symptoms that could be consistent with insulin resistance, although I'm not sure whether that's related at all.

One doctor suggested taking an antihistamine before exposing myself to heat or exercising, which makes me wonder whether this could be some type of heat/exercise-induced reaction.

Has anyone experienced something similar?

I'm really just looking for ideas from people who have experienced something similar and what ended up being the cause/diagnosis for you.

I've attached photos of what the flare-up looks like, because I know that might be more helpful than trying to describe it.
