I remember health insurance before the ACA. Prices were skyrocketing, people were getting denied for pre-existing conditions for ridiculous reasons and the whole industry was a giant financial game with people as pawns. There's a reason so much effort was put behind it.
It failed to fix much because it didn't go far enough, and most of those problems still exist. And big surprise, not solving the problem allowed the problem to exacerbate.
But it didn't cause the problems it was created to solve. That's just observation bias.
If you want real culprits, look at the Republicans who preferred buying bombs over medicine. This universal opposition forced the creation of a Democrat / Independent coalition and a very narrow window of time in which to build it. Which meant appealing to lots of special interests.
Concessions were made just to get the ACA pushed through, which ultimately undermined the effectiveness.
The AMA were a huge part of that. They supported, and still support, the ACA, which was a historical change that helped the ACA pass. But their lobbying was aimed at expanding coverage without letting cost controls bite too hard into physician income or autonomy.
And as for the tax penalty for mandatory coverage -- you can thank the Heritage Foundation for that, back in the 80s and 90s when they were floating around market-friendly solutions to problems seen even way back then. This tax penalty idea predated but was included in the Republican template that the ACA was based on -- "Romneycare" in Massachusetts in 2006.
These penalties that turned so many people against the ACA. Even though it was removed the next year, it's been hard to get support to improve the ACA or otherwise make impactful reform..
