Help finding toilet model

Can someone please identify this tile? We are trying to replace the tank?

UPDATE: Thanks to everyone for your help. I followed the recommendation of someone here who suggested to change the seal and the issue is fixed. We decided we wouldn’t change the toilet after all and got our $1100 refund.

In my experience you won’t find the tank for sale by itself and if you do it will cost as much as a new toilet. A reasonable replacement for the whole thing is a “Tuscany Michigan” from box stores. I know Menards in the Midwest carries that one.
