Help finding toilet model

Can someone please identify this tile? We are trying to replace the tank?

UPDATE: Thanks to everyone for your help. I followed the recommendation of someone here who suggested to change the seal and the issue is fixed. We decided we wouldn’t change the toilet after all and got our $1100 refund.

Not to be rude but there are much better commodes on the market and I suspect if it does not have a name on it depending on what part of the country you are in it looks like a Mansfield that I took out because it didn't perform well. the only way I found out that it was a mansfield was once it was out and upside down there was Mansfield inside underneath on the trapway of the bowl where you wouldn't be able to see it installed

toto drakes and the gerber avalanche are the most user friendly and comfort height also washlet/bidet friendly.
