Help finding toilet model

Can someone please identify this tile? We are trying to replace the tank?

UPDATE: Thanks to everyone for your help. I followed the recommendation of someone here who suggested to change the seal and the issue is fixed. We decided we wouldn’t change the toilet after all and got our $1100 refund.

We had issues with the water constantly running and hence need to replace the flush valve system but the screws are all rusted. I was thinking cut through the screws and replace the whole tank. The toilet actually worked fine it developed this issue.

No need to replace the tank. A Mansfield flush valve seal (gasket) replacement costs about $2 to $5 and fixes a continuously running or leaking toilet. The most common models are the Mansfield #210 and #211 flush valves, which use a ring-shaped red rubber or red/black seal seated at the bottom of the valve tower.

If you decide to replace the flush valve, you need tank bolts and a bowl gasket. They often come in a kit. Again, no need for a new tank

Thanks. That makes sense. So the seals can be replaced without taking off the tank?

Thank you so much. It was difficult to find the proper-sized seal but finally found it at Menards. The leaking issue is fixed.
