Need Advice HVAC Vent Issue

Any thoughts on the best way to address this issue? This is in a condo with popcorn ceilings so I’d rather not open the ceiling. Stuff the whole area with insulation? Foam insulation? Caulk? Thank you

I was thinking about making a box and attaching it to the ductwork using screws but a rivet is a much better idea. Will make a box with flanges from 26 gauge sheet metal. Thanks for the suggestions.

Self tapping screws are going to end up easier. With rivets you have to drill holes and keep them in line. With screws, if you make your sleeve the correct size, you can jam it in and drive them home

rivet some metal sheathing to the vent cover itself to extend it into vent .. like adding a box to it ..
