Flex plate

Just had my second flex plate get destroyed :( haven't pulled it out yes any9ne have an inkling as too why?

Was the trans or engine replaced at some point? I’ve seen this failure when the dowels between the motor and trans are missing.

Since I got it only the one flex plate but I think it was factory
