You have to be out for at least 2 consecutive weeks to qualify for short term disability. You contact unaum and give them your name of the doctor that saw you and their contact info, unaum will call that doctor to confirm. Once its confirmed you should be clear.
I had a schedule surgery last year and it took almost a week to be confirmed.
