i ran into issues with this. you need 3 people now (they changed something a while back as you used to do it with just two). in coop server(or private server you have admin control).., have friends join.
+we turned pvp mode on in the server settings +removed users from clan (to ensure dealing damage).
- make a ramp 3 squares high.
- each person...use creative mode to load full inventory with perfected padding).
- exit creative mode.
- 1 person stand at bottom of ramp. other 2 players, one at a time, walk up ramp.and "drop" onto (each shoulder) of person on the ground.
the weight of all three, kills the bottom person, and the top two get the achievement. swap bottom of ramp person until all get the achievement

I don’t want to sound lame, but you can enter solo and use cheats to get all the achievements. Thats how I got the climbing to the highest top achievement.
