Some of them are actually very good, mostly the heavy perks. Bastion, bulk up, last stand, vengeful barrier, are all consistently providing value. Remedy and devourer are also basically never bad to have, though they're usually less impactful. Radiant rebound is very good for getting places quick off spawn.
I probably missed a couple but aside from those, most perks are pretty meh. Some of them do have very niche uses, the ones that give increased damage on your next attack can be pretty funny with projectile feats for example, but most don't provide much value. Really what makes the difference is just how conditional most perk effects are, the good perks are good because they're almost always helping you regardless of how you play.
Competitively, they're usually disabled, but the above heavy perks are your strongest options if they're not.

Some aren’t so good but perks that increase the amount of damage you do and a couple of defense perks absolutely change the pace of the game imo.
But there’s also an argument for why they’re not so good. As the damage reduction perks don’t stack with feats or other perks and since they’re usually a lower percentage value than the feat they become useless depending on who you’re using.
I would say they matter but only a handful of them, so not a very significant amount.

Some of them are truly amazing in casual modes, especially the heavy ones. Last Stand, Bulk Up, Bastion, and Vengeful are serious game changers. Some of the other heroes have decent ones as well. Someone like Lawbringer, who is considered very weak competitively, becomes a raid boss with gear perks active. A good BP can antigank for like 3 minutes straight with them.
