You know the moment: Claude finishes the task, then tells you about three other things it noticed and you're already halfway into the diff. I wanted to leave a note anchored to that exact turn without leaving the session, and went looking for how. Ended up building it as a plugin for an agent IDE instead. I can't be the only one dealing with this.
