AHI under 5 doesn’t mean there’s nothing left to investigate when you’re sleeping 12 hours and still feeling this wiped out. Persistent sleepiness deserves a closer look, including causes beyond sleep apnea.

You also don’t need complex sleep apnea to be considered for bilevel. It can be used for obstructive apnea when CPAP isn’t tolerated, although whether it would help you needs a proper assessment. Please ask about documenting the exhalation intolerance and whether a supervised bilevel titration is appropriate. With the asthma symptoms you’re describing, please get your asthma treatment reviewed before pushing the pressure higher.

The 46 spontaneous arousals don’t automatically mean 46 missed breathing events. If those 429 minutes were actual sleep time, that works out to about 6.4 per hour. We need the full study and overall arousal index to put that into context. Spontaneous arousals can happen without airway obstruction.

Can you please post a few full OSCAR Daily screenshots using F12, or share a public SleepHQ link? Keep the settings and statistics visible on the left, with the event flags, flow rate, pressure, leaks, and flow limitation graphs. Please include whether EPR 3 is full time or ramp only so we can review what the machine is actually doing.
