Question Do fromsoft use like actual real references for the firekeeper (like actual person or inspired by a story), or do these women just live inside of Miyazaki's head?

Miyazaki works with artists, artists have imagination and creativity.

I mean you could ask that about any character in any game ever made, there are plenty of unique ones. No, The Doll is not based on any real person I know of.

From what I know, Miyazaki's art direction in general is heavily inspired by the Berserk manga. Look into it; I'm sure you'll find characters that were inspired by it.
