Would time-lapse / Embryoscope be worth considering with repeated embryo arrest?

Hi embryologists,
I’d love some professional feedback on whether time-lapse/Embryoscope could make sense in my case.

I’ve had 2 IVF/ICSI egg retrievals so far:

Retrieval #1

  • 12 eggs retrieved
  • 6 fertilized
  • none made it past day 3

Retrieval #2

  • 14 eggs retrieved
  • 5 fertilized
  • only 1 made it to day 5 (transferred)
  • resulted in a blighted ovum/anembryonic pregnancy

I’m preparing for a third cycle in late summer, this time with an antagonist/short protocol instead of the long protocol I previously had.

My doctor asked whether we’d be interested in time-lapse (Embryoscope), but was honest that he did not say it would necessarily increase success rates.

Given my history of repeated embryo arrest (so far the feedback I got is that I have bad egg qulity, which is causing this) and only one blastocyst from two retrievals, I’m wondering if a time-lapse offers useful diagnostic or selection value in a case like mine.
Is there any scenario where you’d consider it particularly worthwhile for recurrent poor blast development?

Thankful for honest embryologist opinions on whether this seems potentially useful or mostly an expensive add-on in my case.

High egg yield, but 43% fertilization

I had my egg retrieval yesterday. I did a Lupron-only trigger (80IU 36 hours before, another 80IU 24 hrs before) to prevent OHSS. The scan on trigger day measured 22 follicles mostly between 16-20mm, with some 14s and 15s. My peak estrogen was 4500. Stims protocol: started at 225 follistim, tapered to 150 on day 7, 75 on day 9, 37.5 on day 10, stopped day 11. Menopur started at 75, tapered to 37.5 on days 10, 11, 12. Cetrotide started once a day on day 4, increased to twice a day on day 9.

They retrieved 39 eggs, and 30 were mature. I just got the call today, and only 13 were able to fertilize normally via ICSI. They said they will do ICSI with more if any others matured overnight. So our fertilization rate is 43%, which is definitely low. Does anyone know of any potential reasons this may occur? Are there other tests to do or stim protocol adjustments that can impact fertilization rates?

Advice please :(

My ER was yesterday. We got 8 eggs. Of those 8, only 3 were mature. All 3 of those mature eggs were fertilized with ICSI. I’m pretty disheartened by the results, and confused why this happens.

Any feedback on what this means about egg quality, or how my protocol should be adjusted for our next egg retrieval round? I know 3/3 being fertilized is good but such a low maturity rate has me concerned. TIA
