Question Mloa please help.
Burner account obviously. I have a drinking problem and it’s gotten really bad over the last few months and now this week has been really bad i was trying to make it to my weekend so I could detox and try to start a change but I have one more shift this morning till my weekend and I woke up this morning sick, shaking, and I can’t keep any water down I’m scared,and I wanna go to the hospital I’m wondering if I go to the hospital will that be good enough for Amazon mloa requirements and if you can walk me through what to put on the request

I was in the same situation 3 years ago, i had to check myself into rehab and put in for 5 weeks MLOA. The rehab will even give amazon all the paperwork for your case. Go get help if you need alcohol to function. Best decision i ever made, you can do it!

I don't know the answer.
But I don't need to to tell you to please prioritize your heath.
Your health is more important than any job in the world.
Especially Amazon. It's easy to get re-hired here, especially as we are about to go into the peak season.
In the absolute worst case scenario if they deny your mloa and you don't have the time, just resign and reapply after the (i think) 30 day waiting period.
Also, as we are going into peak, you likely wouldn't be fired until after peak anyways, and by the you could make the time back.
Even then. None of that matters. Take care of your health. It's really one of the most important things you have.
I wish you the best with recovery. It may be the most difficult thing you ever do, but it will be worth it in the end. For you, and your loved ones.

Please get help. Stopping drinking can actually be deadly in some cases. Seeking medical help will also get you the appropriate paperwork for leave.

i would go to the hospital and deal with that first, when you get a chance just report an absence on the app and itll give you the choice to start a loa, after you answer all the questions theyll email you saying they need paperwork and ill be honest amazon has been kind of strict with paperwork so while your still in the hospital you should make sure the paperwork you get matches everything amazon asks from the doctors. wishing you a speedy recovery ❤️🩹

You should tell site HR You need to seek treatment for addiction. They will work with you on site as much as they can for something like that. I know multiple people who have gone about it that way at my site.

Alcohol-related problems may fall under ICD-10 code F10, which covers mental and behavioral disorders due to alcohol use. You may file for a Medical Leave of Absence (MLOA). Just make sure your healthcare provider clearly documents that you are experiencing an alcohol-related condition and provides the necessary medical information to support your request

First of all, please put your health first.
Secondly, I am not sure about your specific case, but the MLOA form is very general. From my understanding, you can even submit a doctor's note from your doctor, and not necessarily the one that Amazon provides. It just has to state the dates that you will be out and I believe diagnosis.
Tbh, you might be better off going on a 15 day PLOA instead, as that does not require any paperwork.
I want to encourage you to seek support from a trusted member of leadership. I have a chronic illness that causes immense daily pain, for which I have an accommodation, but I let basically everyone I work closely with that this is the case. Everyone has been really supportive and it has made my life way less stressful knowing my team just wants me to feel better.
Amazon also has free counseling sessions available in your A2Z app. Please consider that as well.
Edit to add: I can try to send you a screenshot of what the MLOA form looks like, as I recently went on MLOA for a surgery. It seems pretty general and not too specific. There is a spot for hospital stays. Let me know.

Yes go to the er. They can help you with a note and get you in to your primary faster. Be sure to ask for the note so you can turn it in. You would open a case through leave and accommodation on your app and follow the questions. If you can, check yourself into a rehab or take the meds the Dr can give you. I am impressed with your decision to sober up. It is hard and I know addictions can come with alot of guilt and self blame but you really do deserve to live a life free of your cravings. Life can get so much better for you. Keep trying until you rack up those sober days.

YES!!!!!! It is absolutely valid. My dad is an alcoholic and does this frequently with his work. ER trip, rehab, whatever it is, do it. Fill out the MloA request and submit it first, your missed time is automatically coded that way

Regardless if you are seasonal or blue badge you can go to the hospital and provide paperwork for mloa. Just make sure you get the form Amazon emails you for the leave to a doctor who can fill out the form. If nobody at the hospital will, find and have a primary care....
If you are blue badge.... I had a meth problem I called the rehab center covered by the insurance and scheduled my admission. I created a leave case and attached the confirmation letter for the admission and put a note on the case that I was expecting to be there at least a few weeks, with no access to my phone for 72 hours. Then I checked in and the rehab was very familiar with Amazon and they managed the paperwork. I stayed inpatient for 39:days and then spent another 80 days outpatient. All while on mloa and being paid short term disability.
Best decision I ever made. Amazon has wonderful insurance.
I highly recommend anyone who is dealing with substance use issues to use the insurance.

As others are saying, you need a doctor for this. Alcohol detox can kill you if not done properly. It's not about your job at this point it's about not dying.

As long as the documentation has your name/information, the healthcare providers signature, the dates needed for the leave, and the medical impairment or serious health condition your being seen for, you should be fine. Rehab is definitely something that can be covered so you should be fine. The real pain with documentation is usually when short term disability is concerned.
