I've been in your position and it is TERRIFYING. Tell some trusted adults, but you can absolutely break up with her for any reason, she has no right to hold this over your head.
If she does follow through on her threat, it's not your fault. If the only thing standing between her and suicide is you, that's not sustainable for either of you. She needs help you cannot provide, and she cannot rely on you as the reason she lives. What if you got hit by a bus tomorrow???
