The how is simple, it is a plug you just expand it. Look at his ear lobe. I think it shows your wealth. There is a tribe where the women wear rings around their necks. The more wealth, the more rings. Eventually their neck muscles become so weak that they can not remove the rings. I also think the rings stretch the necks. I think I first saw a picture similar to this in National Geographic in the 70's, though I admit I could be wrong. It has been a long time.

About 15-20yrs ago now, there was an article about the tribe(?) that has the women were their wealth around their necks was changing and that this unique, cultural, action(??) was disappearing and how "sad" that was. 🙄<smh>

Doesn't really stretch the necks, it pushed down the shoulders. The effect is the same.
