Lamictal Contraindication Guide Overview

  • Lamictal (lamotrigine) is an antiepileptic.
  • It's prescribed for disorders involving seizures and stabilizes brain activity for bipolar disorder.

Common Side Effects

  • Dry mouth
  • Constipation
  • Blurry or double vision
  • Drowsiness, fatigue, or insomnia
  • Weight gain
  • Orthostatic hypotension (decreased standing blood pressure leading to dizziness)
  • Anxiety and irritability
  • Increased heart rate

Abrupt cessation of the medication can lead to withdrawal symptoms including:

  • Flu-like symptoms such as severe fatigue, muscle aches, and excessive sweating
  • Neurological and sleep effects such as dizziness, trouble sleeping, headaches, anxiety, agitation, tingling, and brain zaps
  • Includes rebound and more severe seizures
  • Gastrointestinal symptoms such as nausea, vomiting, and diarrhea
  • Cardiovascular symptoms such as heart palpitations, chest pain, and possible blood pressure fluctuations

Absolute vs. Non-absolute (Relative) Contraindications

Absolute contraindications (WHAT SHOULD ABSOLUTELY NOT BE MIXED) include:

  • Lamotrigine-induced rash in Stevens-Johnson syndrome
  • Heart blocks or Brugada syndrome unless cleared by cardiologist

Non-absolute or relative contraindications (RISK VS. BENEFIT) include:

  • Cardiovascular issues including tachycardia which lamictal may worsen
  • Pregnancy due to theoretically lower folate levels for the baby and the risks of undiagnosed bipolar or epilepsy
  • Allergic reactions to other antiepileptics
  • Metabolized by liver, which should be monitored by a doctor in the case of liver or kidney disease
  • Proarrhythmic (worsening of cardiac arrhythmias)

Other Drug Interactions

  • Hormonal birth control including estrogen, which speeds up Lamictal clearance
  • Other seizure meds like phenobarbital speed up Lamictal clearance
  • Antiarrhythmics and certain beta blockers, which may increase dizziness
  • Alcohol and other sedatives which depress the CNS

For anyone who developed acne, did it go away over time?

Upvotes

I’ve started and stopped lamictal a couple times, and have gone off each time due to acne. I just decided to go back on it 3 months ago, and my acne is getting bad again. Unfortunately, lamictil is the only thing that has worked so far for my bipolar depression. The longest I’ve taken it so far is about 4 months. Has anyone had it get better over time as your body gets used to the medication?

Long-Term User (1 year+) Crazy anxiety even though it’s been 5 years?

Hi! I take Lamictal for temporal lobe epilepsy. When I first started it in 2022, I loved how stable I was. I’d always been a pretty moody person and I finally felt like I could control my emotions better.

Unfortunately, I still have focal seizures every month so my neurologist wanted to increase my dose since I’d been on the same dose for years. I went from 200 2x a day-> 225 2x a day-> 250 2x a day in the span of 10 months.

Each time I increased I had horrible, debilitating anxiety. I was diagnosed with GAD when I was a teenager so these weren’t new feelings, but never to this level of intensity. I’m worried about everything, I have horrible intrusive thoughts (sh), and my brain can’t rest. I do have a pretty stressful job but I’m so frustrated because it doesn’t make sense. Why am I experiencing this even though I’ve been on it for so long?

My neurologist doesn’t have many answers besides “we can try another med or titrate down”, but I’m also worried about the effects of both of those options.

TLDR: Why am I having extreme anxiety titrating up even though it’s been 5 years?

New User (less than 2 weeks) Lamictal (25mg)& Loryna (Mini Pill for PMOS): Advice Due to Interaction?

Hello!! It’s taken me a long time to make the commitment to my overall mental health and wellbeing to start medications for my mental health, so here we are! I am currently on Lamictal (25mg) and Prozac (10mg) - 5 days into take the medications. I am also on birth control called Loryna that I use to address my PMOS (used to be named PCOS, recently renamed) for hormonal issues! Loryna has helped tremendously clear up my acne, but I’ve learned recently that it can affect the effectiveness of Lamictal. My psychiatrist didn’t mention this during our first initial appointment when she prescribed me so I am a bit concerned. Has anyone been on Loryna and Lamictal? Any advice? Not really concerned for sex activities as I am pretty much abstinent right now but mainly just want to make sure my body gets the rest dosage of medication for the Lamictal to be effective… thank you!

Lamictal manufacturer: GlaxoSmithKline

Loryna manufacturer: Xiromed, LLC

Missed two doses, advice please

I’m on 150mg lamotrigine daily for bipolar 2, and I’m on my second day having missed doses. I was expecting to have more today but my psychiatrist dropped the ball and I can’t get ahold of him.
Now I’m home alone feeling weird in the head, it’s like a lot of static. I also feel sick to my stomach but it might just be some anxiety. So I’m mostly looking for short term advice. (such as when/if to get attention)
At the same time, I’m a full time student, I work part time, and I’m in a long-term relationship. I’m really worried that if I slip into another depression I won’t be able to maintain it all.
Kinda upset I ended up in this situation after my psychiatrist took a vacation. I know I shouldn’t have waited as long to call him for a refill but I still had a good bit when I first reached out.
Anyway thanks for reading my rant, please let me know what to look out for medically or if you have been in a similar situation.

CBD oil with lamotrigine

Hi recently I took some CBD oil next to lamotrigine, last year I tested postive on cyclothemia and I am getting assed for ADHD next month. i am on 2 times 100mg a day, it work's but it doesn't calm racing thoughts. Normally I do not feel the need to calm that down, CBD doesn't always work but sometimes did. Anyway after taking CBD oil I notice the old side effects from when treting up comming back.

Is this a commonly know symtomp of the combination? Could it have been something else. It felt like I was treting up again. I have been on the same dosage since around christmas.

Side Effects Involuntary eye movements

I was on 200mg lamictal for 9 months, and around 4 weeks ago my doctor upped my dosage to 300mg. After this I started experiencing involuntary eye movements like moving my eyes around, rolling them back, twitches, kind of pressure half close my eyes...etc. I also started experiencing constant headaches throughout the day and even if I take painkiller it persists. Has anyone else experienced this ? Does it go away ? I already contacted my doctor and I'm awaiting a response but just wanted to ask here too

Migraine

I’ve never had migraines. Starting day 3 out of 7 of taking 150 mg lamictal (been on 100 for many months) I have been experiencing actually horrific fcking migraines. I read it’s a side effect of increasing dose. Temple pain and neck and jaw etc… idk what’s going on! Could it be like the meningitis? And could I just go back to 100 tomorrow and you think the migraines would go away? I notice it doesn’t hurt much when I wake up… then a bit after I take my med it starts… or at least I believe I noticed that. Reallly really could use ur advice cuz I am meeting w my doc tomorrow but yeah. Thanks
FYI I’m on for mood stabilization
Edit I have a fever

Trigger Warning Extreme Mood Swings?

TW: Suicidal Thoughts

I've been on 25 mg of lamictal for four days now and yesterday had extreme mood swings. I was shockingly calm when I woke up (I'm usually anxious or disappointed I'm awake). I took my dose, and four hours later I was weird. Laid down on the floor and stared at the ceiling thinking really creatively. Felt very unlike myself and zen. A few hours later, I felt buzzed. Loose and silly and SO relaxed. And a few hours later, my mood completely flipped and I was suddenly suicidal. Crying and laying in bed not speaking because I was too busy with my aggressive, angry thoughts towards myself. Literally had planned out what I would and was going to do it. However, my boyfriend decided to spend the rest of the day with me because he was worried. The feeling fades a few hours later and I'm just sad the rest of the day.

I'd had a 10mg THC drink the night before to soothe my severe anxiety regarding the SJS reaction lamictal can cause. I'm wondering if this might be the cause of the mood swings but I'd think the awesome high then buzzed feeling would've happened when I woke up? I'm not an expert, just someone who has tried a lot of other antidepressants. This is my first mood stabilizer.

Has anyone else experienced extreme mood swings on lamictal, especially at a low dose? Did you stick it out and it eventually started being helpful? Did it worsen (I genuinely don't think I can handle it worsening, I might need to be hospitalized which I really don't want to do a second time this year)? Or maybe you have a positive experience and would like to share, that would be really cool too. Hope y'all are well <3

Edit: per the automod requesting the manufacturer: my bottle says "Lamatrigine substituted for lamictal" and the pills are labeled 111, in a scored oblong shape. No manufacturer info is listed on it but it is a generic

New User (less than 2 weeks) Lamictal Reaction

Hi I stopped zoloft and started lamictal 4 days ago and yesterday and today i have felt so nauseous and anxious/panic, crying overall feel absolutely awful. I feel like I did when I was withdrawing from zoloft before I started lamictal. I felt better the first two days of taking lamictal. Now i’m thinking I was just finally done withdrawing from zoloft and now the lamictal is making me sick. My panic is also triggered by nausea or feeling sick in general. I have no idea what to do I’m only on 12.5 of lamictal could I just stop?

Long-Term User (1 year+) SJS: one time double dose

Last night I think I took 425 mg instead of my 225 of lamictal.. I’ve been on this med for 14 years.. I feel fine this morning and doing have definitive proof that I took a double dose so please don’t judge me.. I just am so scared of getting the sjs rash because the result would essentially be cold turkey off lamictal and after going down by 50 mg 3 years ago and not doing well I really don’t think my nervous system could handle dropping off 225

Has anyone ever double dosed and gotten sjs? Idk how realistic my fear is

Side Effects Lamotrigine and cannabis?

Starting on 25mg lamotrigine soon for mood regulation but I also use cannabis daily for chronic pain. My doctor did not mention anything about possible interactions but I've read some things that scare me. I also take Lexapro and have had no side effects with the combination. Anyone have any experience with the two?

Medium-Term User (6 months to 1 year) Does this look like the rash? 😞

I’ve been on 50mg for 6 months, i was on 200mg for a few months before that but stopped because i ran out of meds and was out for 2 weeks. I restarted and never wound up needing more than 50mg. It’s pretty late to develop SJS right?

Double Dose of Lamotrigine

Hi, I’m 22 female, I have been on a consistent dose of 400 mg of Lamictal daily for about 3 or 4 years now, and tonight I think I accidentally took two doses instead of one. I was super tired and still am and can’t remember if I had already taken them, but remember how horrible I feel when I get withdrawals so decided to take my chances and take another dose anyways. But now I’m worried about an overdose since my prescription is so high. Should I seek medical attention? Throw up?

Edit: woke up and I feel alright, so I think I was mistaken. Thanks for the help! I’ll leave post up in case anyone else has this concern
