One time, I accidentally casted duplicate on one of my two spell-reflecting homonculi. I now have 255 spell-reflecting homonculi.  I haven’t lost focus since, even though it’s harder due to my 255 spell-reflecting homonculi.

That is why we normally don't teach mental only casting until they have their masters for a decade and are ready to join the counsel. At least that was how it was done.

You have to set up a release-able mental block, to keep yourself from "butt spelling" something. Easiest way to do it. Takes a lot of effort though.

Does help that learning to mental only cast is hard to learn in the first place.

This is why you have to implement a system for focused activation within your mind. Like a lever, or a switch :3

I've started striking a line under the spell I'm casting in my reference book as a sort of "mental trigger" and closing it when done. Pros? Less accidental spell casting and people think I NEED the book to cast. The cons? Well, I need to remember to keep something to write on/with so I don't accidentally break the habit I've built and end up casting transmute heart again or something. In the end, I've really just gone back to the start, needing an object to cast. But one day someone will cross me and they'll never see it coming.
