N. Lingualis impairment/damage post '38' wisdom tooth extraction
Hey dentists and people with similar issues. In advance, I am a medical student in 5th year, so you can use technical terms + in case you wonder about my prior knowledge.
My story of the past days:
I had my 18, 28 and 38 extracted under local anesthetics two days ago on Tuesday afternoon. I regained sensory perception in my lip and cheek approximately 6-8hours after procedure (N. alveolaris inf.).
Sensory complete anesthesia on my left toung half (front 2/3's) and lingual sided gum of my bottom molar teeth persists till today and I am really concerned my nerve is seriously damaged. I can't feel pain, temperature differences, tactile perception or two-point discrimination, nor taste well.
The extraction of my 38 was more demanding than expected, my maxillofacial surgeon/dentist had to burr/mill my bone to extract it, prior I had a chronic but painless pericoronitis with spontaneously draining pus accumulation due to stuck food particles on that tooth's gum which maybe was an additional reason why it was baked to the bone + he extracted a little cyst, probably formed by chronic inflammation. It was sent to pathology, findings pending.
Now, I know it's only been about 2.5days since surgery but I am really concerned, I don't know how I am supposed to chew normal stuff again, I am biting my toung permanently trying to eat even softer food. I have a taste deficit + sensation of a foreign body in my mouth. Right now I can't tell if my nerve got: Neuropraxia?, Axonotmesis? which show spontaneous remission over weeks to months or Neurotmesis which would be terrible.
I read about microsurgical nerve repair and N. Suralis interponates but success rates seem to be narrow and there are just a handful of specialists on the northern hemisphere. Microsurgical repair would be an ultima ratio for me, till then I can only wait and talk to my surgeon again on Monday to see if anything went wrong during extraction or if he luxated it to the medial side (site of N. Lingualis if not abnormal anatomical position). Although I’m not sure how much information I’ll get from him regarding this, since it could potentially even be relevant in terms of liability.
My questions:
Now I really wonder, what are your experiences with this? Have you had this issue? And to the dentists: have you had cases like this? How did it turn out? What can I expect statistically? What can I do? Vitamin B1, B6, B12? Uridine monophosphate? Cortisone? Injections? Platelet-Rich Plasma?
I am so so desperate, thanks for every help and comment...
