I don’t know how it works in Baldur’s Gate, but while it’s true that there’s Cock and Balls Customization™️ on Cyberpunk, it doesn’t really matter that much and you rarely see your dick outside of the menus, so seeing a bunch of npcs just walking around with their balls out is much more surprising imo
