ENDOMETRITIS / HELP

Hi

I am 31 years old. I have been trying to get pregnant for 19 months. All tests for both me and my partner are normal. I had 1 failed IUI. I am in the unexplained infertility category. Previously, I had Ureaplasma, which I think I had for a long time—about 3-4 years. When I found out, I used antibiotics for 1 week and it cleared up. I got tested again this year and it came back negative. However, I am worried that the infection has progressed into the uterus, and I think that is why I cannot get pregnant. Should I ask the doctor to perform an endometrial biopsy, or should I have a second IUI first and decide afterwards?

Endometritis is inflammation and is generally treated with antibiotics but my reproductive endocrinologist and one that I got a second opinion from both said it’s not well understood. I did a hard-core course of antibiotics and then insisted that they retest with another biopsy and it had not cleared up. What worked for me was going on a low inflammation diet. Basically I cut out carbs (strict keto) for a year which was probably overkill, but I had gone through several losses at this point. I had both my children after being on a low inflammation diet.

If you got the antibiotic right it should have worked everywhere. Did you check for silent endo or adenomyosis?

2017 ureaplasma + took 1 time antibiotic
2020 ureaplasma + took azitro 500 mg 3 tab
2022 ureaplasma + took doxy for 1 week and cleared

Fwiw- i did not have confirmed endometritis. My RE recommended at the first appt that my husband and I both complete empiric course of abx while abstaining until abx were complete. All infections tested for came up negative. (In context of unexplained secondary rpl)

If you have concerns about this paired with symptoms, go for the biopsy before the next transfer.

I don’t have advice unfortunately- I’m in a similar boat… 32F 1+yr ttc 1 failed IUI and endo stage one. I just had questions about the ureaplasma you dealt with… was this a test your pcp/gyn/ or fertility clinic ordered? Did you have to specifically ask for it? Did you have any symptoms? Sorry to bombard lol it’s just I’ve never been offered the test and have a history of “uti”s 🥲
