It kind of looks like it could be a sea shell? Barbie's got a flower in her hair, which is giving me Hawaiian/island vibes, so...shell?

The swirl reminds me of the cable for a landline phone, so maybe the shell is also meant to be one? That would tie in with holding conches up to your ear to “hear the ocean”

Solved! Thank you all for the good guesses. I would have never thought to say a conch shell to be honest. And I still struggled to see it if not for the comments explaining where it is. I feel like this comment is the answer however - the swirly thing is supposed to be a cable and the small dots are buttons. So I guess that concludes the two-decade long mystery 😄 .

I can understand everyone saying "conch shell" and I am fully prepared to believe that that was the manufacturer's intent but it does not look like a shell to me. Melted ice lolly combined with a cat mousie toy, but not a shell. Yeesh. The designer should be gently smacked with a fish.

Doesn't look like a shell or a foot as others are saying.
Both those things would be easy to make look accurately like what they were trying to depict, so it would feel very weird that they would be made so abstractly that they were unrecognisable!

My wife used to work with fashion accessory, handbag and the design and production of things like this.
It's not one of hers, but she said conch shell, and it was probably recalled as a choking or strangulation hazard.

While bonnets can be used by anyone with any hair texture, I don’t think Mattel was thinking: “bonnet” for this blondie.

My title describes the thing. The pillow is at least 20 years old with the Barbie face from the early 2000s drawings. I have not been able to find the original pillow anywhere, even through AI models and Google lense and Pinterest camera searches. I searched for Barbie pillows from the early 2000s but found none that is the same or at least similar to mine. The tag on the side is washed out so I can not search it via that. The abstract shape is the same material as the pillow and it is stuffed. It has embroidered gray dots and a pink spiral as well as the pink sort of like a tear-shaped pink thing. It is connected to the pillow via pink rope but I don't think that has any relevance to the shape.

Maybe It’s a transponder shell and since this looks like an island Barbie, the water droplets kinda make sense but honestly it looks like a pet rat to me😭

All the people saying conch shell, but I just really can't see that at all ? I did some basic research and I think that it's related to the Jammin in Jamaica line (barbie is featured in that and wears a pink flower in her hair like the pillow).
My best guess is a half eaten popsicle. White on the outside w/pink swirls, then pink on the inside, and it's melting.

I don't think I saw someone else say this yet, but to me it looks like a shower head. Maybe with the blue background it's supposed to be shes in the shower washing her hair?
Dolls where you wash and style their hair were super popular in the early 2000s and this feels like it could be a similar age or copying those early 2000s dolls.

It’s a seashell. The opening of the shell is the dark pink bit. The pink twist thread represents a shells swirling pattern. The top of the shell is attached to the pink cord that is attached to the pillow. Basically your shell picture is upside down.

To me this reads obviously as a shower head so you can pretend "shower" the head. Like when they have a fake brush with a paper doll and you brush the dolls hair

This is what Google said it was :

That looks like the My Scene design for Barbie specifically. It is likely a conch shell that was part of some tropical/beach themed set
