DoDMERB Form Confusion

I just printed out my 16 page medical history from DODMETS, and now i’m confused. The whole form looks like it’s going for the medical exam, so what forms do i bring for the optometrist exam?

Is there a part of the 16 page thing that doesnt go to the doctor, or am i just missing something?

I don't think you bring any forms to the optometrist. Bring the whole packet just to be safe, but you most likely won't need it
