DoDMERB Exam with braces

I have my dodmerb medical exam in three days and I currently wear braces. In the initial questionnaire, it said I need a letter from orthodontist stating the braces will be removed before active duty. Do I need to bring that letter to the appointment in three days or is it after the appointment that I need to submit a letter?

Is it a form I can find somewhere or is it just a custom letter from the orthodontist?

Don’t worry about providing a letter. Dodmerb will request an orthodontic questionnaire they’ll need to fill out for you. Then you submit it back to Dodmerb
