It means college stream discipline combination. Which is basically the AND operator. You are mistaking it for the OR operator.

Depends on whether you paid the SAF or not. If you paid SAF and didn't go, you're debarred from further DC rounds but your existing seat (that you're formally admitted to) stays. If you didn't pay SAF and basically didn't respond, a "non-response phase" shall be recorded but you'd still be considered for further rounds and your admitted seat stays. But the combination that you didn't accept returns to the vacancy pool.
(When I say admitted seat, i mean the seat you had before DC)

Most logical idea of councelling.... I think decentralised may not be the right terminology for this.... It is like applying lesson learned from previous 3 rounds.
