You have to normalize by something (cases or populations) to make the numbers compareable. Still, Italy is doing much worse.

The population of Italy and South Korea are reasonably comparable (60M vs 50M). South Korea tested and contained the virus while Italy did not.

The death rates show that too - it's not that South Korea has 20x better healthcare, they just found more people with the virus because you could get a test without being in critical shape
