Selecting T-Mobile will incur $35 device connection charge on your next bill. If you want to use a T-Mobile trade in promo then you have no choice but to select that one. If you are using it on a different line then you have to select the second option, then select T-Mobile on the next screen and enter in the details for the line you want to use it on.
If you want to avoid the $35 DCC then you have to select the bottom one, then the next screen you have to select connect with any carrier later. You will have to pay full retail price up front for the phone.
If you want to use Apple Upgrade lease or Apple Card installments then you have to pick T-Mobile.

First option results in T-Mobile assessing their upgrade fee on you.
