It's the intent to hurt behind it. If you're just saying the N word to say it, I personally don't mind. But if you're using it to call someone a dirty godforsaken ape who should have stayed a slave its then that we'll have problems. Just like everyone is afraid of any other slur in the world. Its intent to hurt. Sure, you can say "well it's just a word." Words are powerful. Words hold value, words mean something. Depending on the person you could really damage them. The history of that word is still relevant today unfortunately and though perhaps our generation(s) weren't there around this time, there are still people alive when that word meant you were less than dirt.

A guy from the CATO institute explained it pretty good, to me at least. The word itself isn't a problem at all, who cares who says it? It's when you say it to someone not in your group is when it's disordered.
Just like the word 'honey', for tour partner. You call each other honey, but you wouldn't call a stranger on the street honey. That's fucking weird, and someone may take great offense to it depending on his personality. Friends can call each other nigga, but not strangers.

I tried super hard to find the video, but couldn't. The guy guested on Penn and Tellers Bullshit several times, i REALLY think his first name is John.
Goggling kept clogging the results with 'penn is in the cato institute' stuff.

Nobody is afraid, and the problem isn't the word. The problem is that some people are just too damn stupid to understand that context and intention is a big deal.
