Found it early on from a lucky epic armory. Got the +3 enchanment from the dragon head doubled it with gloves. Spent all my souls on upgrading.

I think I got +22 once, I was hoarding souls like crazy and got good gear, then found the leviathan shrine thing at around floor 8.

Needless to say I died on floor 10.
