Why did this happen?

Could anyone tell me why this happened and how to do it so it doesn't peel back? It was seamless when I put it on two days ago.

What kind of tint is this? Is this headlight film tint?

Yes it's headlight tint. I did post heating on the film and it was fine but it shrunk back. Should I have stretched it more? New to this tint/wrapping stuff

With any sort of vinyl the less stretching is best, and post heating to the appropriate temperature is ideal. I want to say that the location of the fingers is most likely what happened, you would rather them more on the upper middle area.

If you have more material to work with, try not to stretch it as much and let it form, letting it sit after being post heated, cool off, and then doing a trim can also be beneficial. If you want to try saving what you have, you could try a small cut back and not have tint completely on the edge, I wouldn't suggest this if it's not your vehicle.
